Dr. Mohammad Azim is a board-certified family medicine physician with a special focus on geriatric care and preventive medicine. He earned his medical degree from Touro University Nevada College of Osteopathic Medicine and completed his Family Medicine residency at Penn Highlands DuBois, where he developed a deep commitment to caring for older adults and patients with complex, long-term health needs.
Dr. Azim is certified by the American Board of Family Medicine and brings a thoughtful, comprehensive approach to caring for seniors, particularly those navigating Medicare, multiple chronic conditions, and age-related health changes. He is experienced in managing conditions such as hypertension, diabetes, cardiovascular disease, arthritis, and cognitive health, while also prioritizing prevention, mobility, and quality of life.
Philosophy of Care
Dr. Azim believes geriatric medicine is about more than treating illness—it’s about preserving independence, dignity, and overall well-being. He takes the time to listen closely, understand each patient’s goals, and involve them in shared decision-making. His approach emphasizes proactive care, clear education, and coordination across specialties to help patients age confidently and safely.
He is especially passionate about preventive care for older adults, including wellness visits, medication management, fall prevention, and long-term health planning, ensuring patients and their families feel supported at every stage.
Outside of medicine, Dr. Azim enjoys traveling, reading, playing the piano, and spending time with friends and family—activities that reflect his belief in lifelong learning, balance, and staying engaged at every age.
